I need a sizing label designed for the collar of clothing like the one attached. Needs to be simple in nature

It needs to include the following;
Name of the brand, 'Redesign Apparel'
The website- www.redesignapparel.com
'Designed in Adelaide, Australia'
And I need a seperate one done up for each of the following sizes: XS, S, M, L, XL, 2XL

I have also attached our logo so you get what we look like
