Data entry system 1

I require a .net application for several data entry clerk to be able to manually enter data from a printed form. The form has about 4 pages or questions. The application must be multi user with user administration functions etc. The Database must be SQL2005 and run in IIS. All the data going into the system will be validated at the time of input and the data on the forms will be selectable from a drop down list as the form only has multiple choice questions. Once the data is in the system we must carry out some data transformation tasks and then export the data in to a fixed space delaminated file format.
