asp collection form

i have an existing asp collection form that collects several pages of data from user input using radio buttons,the asp method uses a "trim.request" and a "response.write" method; need to add text boxs into form also so user can input more detailed answers. the form uses an "include" to process the data from the several page form and email to an address. i need help creating just a text box entry and the other required code so i can integrate into form and add as many of them as i would like in the future. so i need help to get up and running with correct code. see sample below, can provide all code and forms if you think you have a solution
