Create app for multiple platforms

I have an idea for an application to service a particular market in Australia. The app would be required to have a form based structure, where the person could add details, for example name, date of birth, address, etc, and that these details could then be used to populate a form which could be printed. The forms exist in .doc or .pdf. I would also like there to be the ability to take photos of people and documents, using the inbuilt camera in the devices, and then attaching the photo to particular fields within the form. Ideally, this app is required for tablets, iPad and Android. In a perfect world this would also then sync back to a website. Users would be on a paid membership basis, for example $x/month. The app is only in development stage within my head so I am looking for someone who can help me flesh out the details and who will guide me in good ways to organise and present the UI.
