Automate clamp design

What we are wanting to do is create a program that will automate our design clamp (end product). currently we have a tech measure the Pipe that is to be worked on. Then he uses our form (attached) and writes down all of the information in the form and use per determined formulas which will then allow him to draw the finished design of the clamp. I am attaching a set of blank forms as an example and also attaching a completed example to work from.
