Hello,
I have a 104 page pdf document with lines in it to indicate where a user should add personal information.
The project is to simply create an interactive form field for each group of lines.
-One of the pages is "Feb. 2014 [url removed, login to view]" (see attached)
-Virtually all of the other pages look like "[url removed, login to view]" (see attached) and should be easy to edit since they all look more or less the same, and each two-page section needs only one or two fields
This should be a simple project, but in any case please specify how many rounds of revisions you are including in your fee in case revisions are needed.
