Capture name & address from CSV data

The task is to capture name and address data from about 700 CSV records. The final data set needs to be in the form: firstname,lastname,alphanumeric address,zip+4. There should be only one comma between fields. Extra commas must be removed. When there are two first names use the man’s name only. When there are two owners with different last names, choose the first person listed. Remove all initials. All data after the zip code is to be discarded. Records that have addresses outside the USA , if any, should be discarded.
