Clone Simple Facebook App

I need an app that has the ablity to; 1. Add a custom tabs based on - if the user is a fan or not: - If the user is a fan: Tab3 is displayed - on this tab there is a form, where the user type something in, and it gets submited to the form on my webstie. 2. Then on Tab2: just an image and the ablity to display diffrent content to a user depending if they are a fan or not.
