Customise Themeforest into new design - repost

We have purchased the following themeforest template (4 menu items) [login to view URL] and want it to be customised as per our supplied .PSD artwork. The functionality is very much the same with the exception of a web form that must be fully functional and look visually as per the .PSD we supply (you need experience in the same) and must validate server side.
