CD Design and Artwork

We're seeking graphics for the printed image of an audio cd. We have laid it out crudely in a word document so the chosen provider will replicate this in terms of the layout. 4 image files will be provided, including a high res photograph, as well as the business log. The images and text are to be laid out on a plain white back-ground. Space permitting we may want to add twitter and facebook icons to the artwork also. The final artwork must be: Minimum 300 dpi (CMYK) as this will be printed as thermal print on the CD surface. File types we will take: PSD (flattened) - PDF (flattened) TIF (flattened) - JPG Source files also to be submitted.
