Create custom map of North America with 95 markers using approx 15 custom icons (each custom icon represents a company) and legend to match... - repost 2

I have a database in excel of approx 95 marker points. These points represent approx 15 companies in total. I want to create a map of US & Canada importing the data points by address. For each of the 15 companies I want to use a custom icon marker on the map. Then I would like a legend which has each of the 15 companies listed next to the custom icon marker. The address database is complete so all that is necessary is to import the data points in the map. I have created jpg files of all of the custom image marker icons but 2 of them. Since they don't have a logo, it simply can contain the names or abbreviation of the companies. The important part is that the legend on the map match each custom icon marker with the full name of the company. I was using the software called imapbuilder but the software does not allow you to create a legend with custom icons. I am not looking for a flash/interactive map. Simply looking for a map that shows all the markers with custom icon markers on the map and a legend. I don't want states to be identified on the map because it will appear too cluttered with 95 markers throughout the US & Canada. I would like the ability to modify the project down the road if necessary after the project if I have a couple of minor changes. Please provide your completed data file and identify which software used so that I can hire somebody to modify this project down the road if you are not available. This prevents me from starting over. The finished project needs to be displayed using a common format via email in a pdf, word or other common viewing document. I need this to look professional and have done most of the leg work in creating the database of companies as well as all but 2 of the jpg custom marker icons. Now I believe is the easy part of importing the data and putting it on a map. My budget is $30. I would like to know when you can complete the job. Let me know if you have any questions.
