Create a form in Filemaker Pro

I have just recently come across Filemaker Pro and think it could be a great benefit to my business. As such I would like to convert some of the paper forms we use to Filemaker Pro so that I can then use them out in the field on the iPad.
I have attached a copy of the form that we currently use. I will need you to go through and recreate this in FM and have it useable on an iPad.
To prove that you have read this description and understand what I am after, when bidding on this job I need the very first line to tell me what year we are currently in. To give you a hint the answer is 2014.
The task will be paid on an hourly rates and there could be the potential of additional work, so as such I will need you to have freelancers Time Tracker installed. Also once awarded all communications for the task will be done through basecamp in which I will set you up.
