Create a PHP script to interact with an SMS API

We have a MySQL database with uk mobile phone numbers, account information and dates. We require a PHP script that will automatically send a pre generated text message using the zensend API ([login to view URL]) 4 weeks, 2 weeks and 1 week before the date in the MYSQL row.
We also require a simple sms sending interface built using the same API.
The database contains the following information:
Customer name
Customer phone number
Customer code
Customer dealer
Customer date
We need a simple script that will send the customer a text message to their number that will include their name, code and dealer, 4 weeks, 2 weeks and 1 week before the date in the row. There must be a simple backend to allow us to customise the message at any time.
The script must also log whether the sms was successful or not. If the message was not successful, it should disable the customer from receiving any further sms messages.
