Create a review module - Prestashop 1.4

Hello, I need a customer review module for my prestashop store that is build on 1.4. 1. When changing order status to "Order sent" in the email the customer gets a link to provide his/hers opinion on the purchase and when she done it he/she will get 10% generated coupon code that can be used 1 time. 2. The link goes to a page in the shop with a form where they choose: 2.1 5 stars to mark how many stars. 2.2 First name, Last name, City, 2.3 His/Her opinion in text. (text box) 3. Send button (opinion is sent) 4. In back end I can see every opinion and Approve/Edit/Delete 5. In the front end it should be a box on the right column that looks like the uploaded image. 1. Stars shows and filled out (how many chosen) 2. Date the review was written (not posted/approved) 3. Topic 5. Opinion text 6. First name, first letter in Last name and City That's all, pretty easy forward module! Budget is 100 usd. The module should work on Prestashop 1.4 and you work on your server, you deliver a finished product for install.
