Diabetes Email List Development

Our business makes products for Diabetics in the United States. We need to constantly get fresh leads of USA Diabetics. We don't try to always sell them our healthy products right off the bat, typically the first conversion is that they can request a free sample kit after filling out a survey. Regardless of which landing pages we are sending them to, I need a consistently fresh source of diabetic leads. I'm open to multiple methods of gathering the data, but I am not interested in purchasing a list that's been re-sold many times. The budget for the project is really just to introduce us to a possible solution provider. The ongoing scope and budget is much larger once we have a process in place.
