App to generate PDF docs with QR code-type image from codes in CSV file

We need a Windows application that can generate multiple PDF documents based on the contents of a CSV file. A single page PDF must be generated for each row in the CSV with each including a black and white vector-based symbol whose composition varies depending on a 24-bit number - the ID code - included in one column of the CSV. The symbol is like a simple QR code with black squares that need to either be present or not depending on the value of a specific binary bit in the ID code. The other columns in the CSV carry text which must be included as basic text in the PDF. The application should be written in code that could be recompiled for or run on Linux and other platforms (perhaps C++ or Java), and the source provided as part of the deliverables. Perhaps an open source library - such as Haru, Batik or The Processing Foundation’s PDF Export - could be used to create the PDFs. A more detailed brief, including a sample CSV file and PDF templates will be shared with suitable candidates.
