commission calculator from excel to web application or pgrm

My boss created a commission calculator in excel and I need create a program or web based access php application (advice on which direction to go) that uses the same information. There is variable prep work that can only be changed by a manager then there are results that are prep and fixed (entered by the user) and then the calculations are populated, with one cell that still needs a complete "IF" statement. And the commissions are from 4 different variables. 20 total fields with three columns (period 1, period 2, total) So you need to take the excel sheet that I provide and create a software program or web application.
