Android Mailing List App

Budget is 300$ We would like to get an Android Application developed. It should be developed for tablet however if it can work on both tablet and mobile it would be great. It is basically a mailing list sign up app that will allow our customers to sign users up on their mailing list at events electronically instead of using paper forms. I have attached a rough draft of the app including a document outlining the features (we are also open to suggestions that would make this app better). We would like to have this draft enhanced and highly polished into a modern user friendly app. We will need app icon and a loading screen background for app with logo. We would like to be able to install this app locally as a download from our website or as a download from the app store. The application should be generic enough that it can be customized to various individual businesses. To be considered for this job please attach samples of previous apps developed especially if they are similar in nature.
