Ajax dynamic form

I've a form where I want to add some extra features. A.- Image/File Multi uploader. I like the one that I found at: [login to view URL] but I need the follow modifications: 1.- First of all I want 2 instances of File Upload inside my main form. One for Images and the other for files. I know that I can have just one instance, but is better to have 2 seperate. 2.- When the file appears in client side it shows the thumbnail, filename, filesize and Delete button. What I want to be changed here is to have the thumbnail, an input text field for title, a textarea for a short description and finally the Delete button. I also want the thumbnail to be resized and then croped to a given dimensions. 3.- After upload I want 3 image versions to be save in the server. a] Thumbnail: Resized then Croped to a given dimension at .../photos/thumbs/ b] Midsize: Resized then Croped to a given dimension at .../photos/banners/ c] Fullsize: Just Resized to a given dimension at ...photos/ 4.- Using PHP and MySQL at server side I want the follow data to be save in database: a] Article id. This id exists as hidden field in my main form. b] Title c] Description d] Filename ... Filename should be like: $filename = md5(time() + rand()); 5.- Non image files should be upload to different directory and save to different table. B] A dynamic list of Questions and Ratings. 1.- At page load the list should be empty and a button "Click to add a Question" should appears. When the user clicks it, I want to add a row with a text input and another input with a slider for rating. You can use simple slider or some star rating sliders that exist on the Web. What I want is that when the user moves the slider the value of rating to appears as tooltip. 2.- Save to database: a] Articleid b] Question c] Rating 3.- On the right of each added row to be a delete icon (or link) so the user can remove the row. C] Just like "B" but simplier. 2 Input text fields.
