ajax / php quick contact form

Hi there looking for a good coder to help me with a quick contact ajax form see attached visuals pretty self explanatory, neds to be self contained and use ajax / php so no page reload want it to look exactly the same as this so using images for everyhting except the actual form field text if the user submits form without entering in all fields, error text appears inside relevant fields while keeping previously entered data in others eg if person doesnt enter name but enters everyhting ele then it comes back with just error messgae in name field, all other fields display info already entered clicking error field will then blank the field allowing them to type in data when all fields are filled out, form will go to process screen and send form data to an email address supplied, the ajax loader graphic is here [login to view URL] once successfully processed will return to success page along with tick and thanks message there needs to also be a button ink to @recent work@ looking to the inser this obviously into a site so need everyhting nicely seperate inc any css to add to main css etc thanks!
