My client is a mental health service that takes referrals from external agencies. They need to create an online referral form (4 pages) and to have the data stored securely in a mySQL database. The form has fields that need to validate data against tables. There will also be other validation required. This will be specified. We prefer to use AJAX style validation. Once the referral is complete the referrer will press the submit button which will attempt to save the referral and if successful email the referrer. It should also be possible to have a print view of the form, including the completed form to allow the referrer to print a blank form for completion by hand. Security is tantamount due to the sensitive nature of the clinical data collected. There may be some minor changes to the form following on from the original project.
