Custom Link Checking Script Needed

I need a custom script, that you can install on my server. Here is what the script will do:

it will have a form which the user can input:

1. url of the domain to check the link for

2. a box where we can enter the urls where we need to check the links (we should be able to do at least 50 domains at a time)

After form is submitted, it will show a report in tabular form:

Sr. No. URL where the LInk Was checked PR of the URL Whether Link is Present (Yes or no) Anchor Text of the LInk (if link is present) PR of the home page Cache Date of the LinkPage

Please let me know if you can provide a reliable script made in phpt, ajax or any other plateform. My budget is $30-$50
