Create web form using information from Microsoft Word

I have an application and reference form in Microsoft Word format and it needs to be converted to an online electronic form. The form needs to have the following features. 1. User creates a username (his email address) and password to create an account where he can manage his application form. 2. User can save partially completed application for completion at a later time. 3. User can upload documents to attach to his application 4. Completed form will be emailed to admin with attachments 5. Electronic form will be available in English or French. User will select his language I have a attached the English forms that need to be converted. French forms will be provided after I choose the freelancer.
