I need to have code for a web form that I can place on a php web site to allow users to enter a code and see if they have won a contest based on scratch-off tickets they will receive at an event. I need to collect the following information from each person who visits to validate a scratch-off code: First Name Last Name Telephone Number (Optional) E-Mail Address Theatre Location Enter Your Scratch Off Code When the user hits "enter/submit," I need to capture the above information that he or she has entered into a database. Then, I need the browser to redirect to a new web page based on the alpha-numeric code that the user enters. I will also need a database created that will allow me to enter all of the alpha-numeric codes, location (city), and the web address redirects. I also need a yes/no option to have an e-mail sent when a code is entered (some codes will require an e-mail, others will not). I will want the default to be no e-mail is sent, with the option to turn on the e-mail feature. For example, if I enter into the database that code x123y is supposed to go to [login to view URL] in concord, I will enter into a database that is created for this purpose the "x123y" "[login to view URL]" and "concord" information. Finally, if someone enters any code that is not in the database, I would like the program to redirect them to a page I will create to announce the error.
## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).

## Platform
in all 5.0+ versions of Internet Explorer and Netscape, 1.0+ versions of Firefox UPDATED: There is a change in the platform for this project. Instead of mysql, we will need to use MSSQL 7. PHP 4.4.1 is currently installed on the machine.
