Creation of brand new HTML survey form(repost)

RUSH JOB - This project is due by March 20th and was started by another team, who can no longer finish the job. The current project is located at: [login to view URL] user and pwd is "gusti" I am looking for an experienced web programmer or team to create or finish an HTML based online survey form (has about 50 screens) - HTML/JavaScript based linking to MY SQL database backend. The web site is an online form that is filled out once per year and records numerical and text information on various questions relating to recycling matters. The front end needs to resemble an existing sister site that uses a Flash interface. Both the sister site and this new site will integrate into a MYSQL database using ASP scripts. The sister site has been running for several years and members of my team have managed the programming team each year. My client wants to keep much of the look and feel but feels that Flash isn't the technology for them going forward. They insist that the new site be constructed in HTML/JavaScript. We require a senior level web programmer as several of the screens have extensive objects, dynamic lists etc.. My team has a programmer who can do the job, however he is tied up on other projects but can assist if required. My client has also requested that the site be able to print to a PDF file format. THIS IS AN IMPORTANT PIECE please confirm that you have experience with this. The project is set to start in early January, 2005 and run until Feb 10th, 2005. The successful bidder will need to prove his/her expertise in showing sample pages that they have constructed. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform This new survey needs to be able to run on MS Explorer 5.5 or higher, Netscape 6.0 or higher It will integrate into an existing MYSQL database which my team will manage. My team will do the database changes in working with you.
