Add CAPTCHA To Existing PHP Form & Fix

This is a VERY simple project - so please don't over bid. We have an existing website and the form is complete. All we need is a really nice CAPTCHA area to the form to stop spam registrations. We will provide the existing files for the form only. You agree to update the files without any need to connect to our server, hosting provider, etc. You agree to the following: 1) Add the CAPTCHA to the existing form, keeping the same layout to the form 2) Add the CAPTCHA section at the end of the form 3) Enter the code in a clean manner with "Comments" to the HTML code - for future modification and/or editing of the CAPTCHA code 4) The CAPTCHA code must be modern and the latest CAPTCHA security 5) There is an issue with the AJAX / javascript that needs to be fixed for the "Submit" button on the form. (When the form isn't completed with the requested fields and the user hits Submit the form disappears.) You need to fix the Submit verification. 6) Use our test server. 7) This is a FAST turn around and need it within 1-2 days. Thank you
