Add Contact Form (HTML/PHP) to single page.

Hello, I am using these one-page template: [login to view URL] I need someone who can create a new page with a contact form similar to the design I have made myself here. [login to view URL] As you see, you dont need to do much, just remove the header area from the orig. template, add the form codes to the white section and setup a php-mail script that will email me the information the user types in. After succesfull send, a redirection to new URL should also be made (I will just tell u which url and thats it). So very simple stuff for someone with little experience in html/css/php. I will send you the complete template for you to work with. This is 30 minutes of work MAX for someone with little knowledge. Bid accordingly. I want this done ASAP.
