Basic Login form with one time use

We are looking for a basic login form with a specific validation and push to a desired link upon submit. Validation Criteria: Mysql fields (email, lastfour digits of credit card, and a status field) The database and registration form have been created and need the login in form to be built as follows. 1. Login(email) 2. Password(4digits(last four from credit card) Validation rules. If email and password valid and status is set to 0, then push to specific link and populate database field (status from 0 to 1) Note: all users by default have a status of 0 upon registration which is already built. Otherwise give one of two return messages: 1. your password or login id is incorrect, please retry again. 2. You have attempted to log in twice , please contact your server administrator. So as follows there is a validation check on the three fields on the mysql database.(email, last four, and status)
