Concrete5 - Extend an Custom Block/Package to show an contact form

We have one module here to manage Events (ProEvents) We need to extend it to show one contact form to user be able to send his guest list (name, e-mail and textarea for guestlist). It's not mandatory to storage the data on DB, only send the data to e-mail, but if possible to also storage it on DB it will be really helpfull.
