Create simple configuration SEO page PHP/MySQL

I need a freelancer to create a configuration for a simple tool to create tags for seo of my site, it display a different page I need a page for configuring a model category with title, model description contained 1 and 2 contained, must each enter either a mysql table it is setup, then another list containing the title of the page, the page type, the status, the slug, the as seo, seo the contained 1, 2 contained the seo, seo description, we must be able to edit, delete or add, and the ability to search.
