Customer feedback solution

We need to simple customer feedback solution, which allows visitor to click happy/unhappy button first, then it shows a screen to select predefined alternatives and a comment section.

All data will be stored on local pc and then send to mysql server. if no connection with database is possible it will try to update the results when connection has been restored.

A admin page with results should be make with filter to pull results for a spesific start/stop date and export to XLS/CSV

We will deliver graphics. page with alternatives should be in html or php + css with prefered jquery.

It should also be made compatible for most screen sizes, starting with 1280x1024 in phase 1,

Phase 2

When Phase 1 is finished there is an oppertunity that we want you to make this solution to also work on mobile phones and tablets. (so Responsive design is needed) but that version can offcourse update the online DB directly without local storage.

This is not part of this project, but please keep this in mind if you would like to develop this as well. (you can give a fixed price if you want to build this at the same time)
