Database system, where i can create my own flat databases

I am looking for a script where I can log into the back end and enter in any fields I wish and what they are in order to create a database front end.

For example if I am creating a contact database I would like to log into the admin/back end click on add field Text field required or not and this will add this to the front end.

Or add a new field for a drop down like title Mr;;Mrs;;Miss etc.

Then on the front I could design the fron by using tags ie. {name} {address} etc. what ever the relevant database field is.

TO give an idea online what I am looking for is:

Click on edit listing template this will give the general thing i want

Databases will not be relational.

Also forgot to add, I also am looking for a way that I can set up filters on the page, so i can filter data by date , free test search etc.

To go over the basics of what I require:

I need a system in PHP and MYSQL where i can add say Name / Title / Job / diairy etc. and the fields need to be configurable to drop downs , field input , var char , blog etc.
then on the front screen it will show the tables i just added and can then add data to them so each table on the from would have edit / new / delete / search etc.

So need all this functionality along with the search facility.

This is so i can make a database with the script you make put it in a folder / database call contact for example then set this up, then do the same with your script and create other databases or my own

Then the front would need to be editable with tags so i can make the page look how i like so tags setup ie/{name} {number} etc.

Also the front would need to be search able so i can search/filter my data on the front end
