I have hosting with hostgator

I need a mysql database set up

I need a simple html form to enter items into the database

Sample entry

DATE (Default to todays date)

SPORT (defined list)

TEAM1 (User Entry)

TEAM2 (User Entry)

PICK (User Entry)

RESULT (WIN/LOSS)

If you do good work, I'll also hire you to built the html report page for this later.
