create a production sheet in PDF format

Looking for someone to create a pdf that can be inserted with dynamic data. Basically i am a Printing shop and Production sheet created which can be typed into and then printed. If its possible with a php page has to be created and turned into pdf and printed i wouldn't mind doing it that way. I have a webserver already using codeigniter php . Things needed are the following: 1.1 Customer Name 1.2 Order Number 1.3 Insert an image which re-sizes to fit 2. Selectable check boxes (Must Be able to select multiple check boxes if needed ) - Screen Printing - Digital Printing / Decal - Embroidery - Heat Press 2. Artwork Size 3. Artwork Colors (Must be able to add or delete color boxes The next area needs is basically a table. I need to be able to add or delete tables top of the table a input box : Item Name/Style There will be 10 Columns . 1. Color 2. YS 3. YM 4. YL 5. AS 6 AM 7 AL 8. AXL 9 A2XL 10. Other I need to be able to add and delete rows in each table .
