CMS page with growing number of input fields(repost)

I need a user profile page that has columns and rows. There are about 25 rows with titles in the columns for days of the week. Each row is a checkbox where people check to show thta they did that activity on that day. Look below. Mon Tue Wed Thur Fri Sat Sun Ate Breakfast X X X X X Ate Lunch X X X X X X Data Input Field The final row will be a data input field where people can put up to 25 characters that become another row. Each item will be a timestamp of something people do every day.
