Custom Access Database

Okay, so I have a database almost complete, but have a few things that are holding me back. Basically, I am trying to convert an excel form into an Access database, and make a form so that my guys can print out the incident reports as they complete them. It should print just like the excel document does. Attached is the form in excel, where the arrow is pointing that's the section I'm stuck on. Sure I can make a spot for 5 names, but what if only 1 is needed....or what if 6 are needed? How can I make the form create more if needed? After the user enters the date, calculate what day of the week is on that date. 2. Fill in the day with the day of the week based on the date field. 3. Add that to the database. So, 12/25/2008 would output into the Day field "Thursday"! Also, I need to be able to print what the user types in my forms without any problems. There should be 1 report per page, but I can't get that to work either. This should be pretty easy for an access guru to help me out. Feel free to change anything or ask questions! Thanks!
