Custom web app needed for local bookstore, Gainesville FL

This website application will be intended for about 15 different users. When a user visits the website, they will enter a unique username and password. The homepage will include an introduction that is unique to the user, as we want it to greet the user by name. Under the introductory will be a fairly simple form. There do not need to be any "checks", but we need some fields to be "required". Under the form will be two submit buttons: one button will allow the user to submit the information and "add another", bringing them to a new, blank form and the other button will signify completion. There may be 1 submission or 50 submissions. Once the user has completely finished, they will be brought to a confirmation page. We need a confirmation number to be shown so the user knows that we have received their submissions. (We need a way to be sure the data will not be lost in cyberspace.) We also want the submissions to be viewable via a PDF so it can be saved or printed. (We have a certain, basic table design in mind.) We will also need to develop a way for the user to log back in and view their orders with the ability to make changes if needed. We will handle all of the language on the website and we know the visual design we want. Nothing fancy needed. It is imperative that we hire someone who is extremely knowledgeable and efficient with this kind of project. We cannot afford to lose this data once it is submitted, and it needs to be perfectly functional so it is easy for the user. We are interested in negotiating a flat fee for the project, being located in Gainesville is important in case we need you to come in and discuss anything with us. We prefer to work with a freelancer and not a company.
