you will need to look at the string and match the field names.
my database name is 'qxdsp' and I would like the table to be named 'regphp' so please use these in your php page. My mysql server is already connected to my webpages via a '[login to view URL]' page so for the connection simply us the 'include [login to view URL]' statement in your php page

1)php page for the above string (named [login to view URL]) which writes to the sqltable in deliverable2
2) mysql table for above . (you need to give me a mysql statement that will create this table automatically)
3) php page to view the collected data as a list showing newest records first and limited to 30 rows per page.
