Clone Printing Upload Form: Print Template Upload Site

I want an upload form pretty much EXACTLY like the one below (layout could be slightly different so we don't infringe on any copyrights) <[login to view URL]> We need it for all printing templates (business cards, postcards, brochures, etc). Preferably we can add our own custom sizes whenever we like in backend. They have Add Spot UV, we want to be able to make that Add Mask File. The mask file will overlay to show where elements like Spot UV, Foil or embossing will go. Needs to be able to upload common print file types. Jpeg, PDF, AI, PSD. The system will send us a print-ready pdf when done with mask files ready to go. Needs to show approval and have them sign-off, same as system displayed. Needs to go to shopping cart checkout, whether that's through our existing zen cart system <[login to view URL]> or another system. We also have [login to view URL] account.
