Asterisk CallBack

Hi, I need someone that can develop a CallBack using asterisk. The callback should work like this: 1. HTML Form that asks the user a Phone Number. 2. Server contacts Asterisk and sends the following parameters: a. phone number of user b. phone number to dial c. user account 3. Asterisk makes a call to the (a) phone number of the user. When the user answers the call, Asterisk play a message (mp3, wav) and then starts the second call to the other (b) phone number. 4. The call has to be recorded and the CDR saved. 5. The trunk to make the calls is a SIP TRUNK that not only is going to make the calls but make the billing. Thanks!
