Create a Web App using PHP and MySQL + jquery and ajax features

I need a web application for manage complex form, users permissions, tables linked to each form and workflow process: compile form (insert/update/delete) saving records in DB and managing users permission process for approve the workflow, manage linked tables (insert/update/delete) etc.. I'll provide my MVC structure to customize and use in this project I'll provide a Word page with all fields (form) structure I'll provide DB structure (MySQL) We'to create this web app using jquery and ajax features)
