A JavaScript QR code Generator and scanner

I need a JS Based QR code generator that works with form fields from caspio ([login to view URL]) and then upload the code into a file upload field. Then I need a QR scanner to scan codes and fill out forms on another page. I would also need the code to be very well explain so I can edit and reuse it in some other projects :P I would really aprieciate any help on this project.
