Automated Psychological Report

I'm looking for a simple way to automate the report-writing process during a face-to-face interview. The user should be able to click decision-trees (Example question: Were there medical complications with childbirth?) and have appropriate (pre-determined) text automatically entered into the report. Example from above: If 'yes' is clicked, a sentence like this would appear: "This individual reported they had complications in childbirth." If no, This individual denied they had any complications in childbirth." The goal is to allow each clinician to generate their formatted report according to their own typical interview questions and preferences for phrasing. The program would necessarily allow easy integration with a word processor along with basic formatting (bold and italics, etc). Again, every user would need to be able to easily compile their own decision-trees as no two clinicians will ask the same questions or phrase things the same way. A second aspect of my needs include a basic level of security to prevent easy copying of the program to multiple computers with just one keycode. That is, I don't need air-tight security, just something that would prevent the average user from passing the program along to their friend with nothing more than their own activation code. I am NOT interested in a web-based solution. This program needs to be native to the desktop and able to run without updates from the web or access to a centralized database. ## Deliverables These attachments should aid in clarifying my needs and descriptions of the project. 1st file is a rough mock-up at the beginning of our process. 2nd file is a sample template report with comments. 3rd file is the rough prototype completed by the last coder.
