Crazy Pet Pictures Tool

We want a PHP/MySQL coded tool that allows the end user to upload a photo of their pet, write a caption about the photo, put in where they are located by city, state and country and allow people to vote about the picture.

The pictures will be displayed four to a page with a next button at the bottom and at the top to go see the next photos. It needs to be viewed both as the newest submissions and the most popular by end user votes. The 3 most popular can even be called out at the top as a promo.

The tool would need to be able to resize the picture that people submit. There would be a backend admin area where the picture can be approved before it is submitted, or deleted out of the database if it is not approved. This tool will be integrated into multiple websites and therefore will need to be able to adopt the look and feel of those websites.
