i need a landing page that requests first name and email address. the page should also have a 3 choice radio button survey that will take the person to different pages depending on their choice. see example: <[login to view URL]> I need a picture link for a freebie on each of the 3 pages. I have the picture. The person would click to receive the freebie and the following would appear. text boxes for full name, address, phone and best time to reach and other info. SEE Example see example: [login to view URL] also, a return thank you note when the person filled out their info and submitted it. and obviously I would need to their information sent to me when they either fill out the initial page or fill out the freebie form and submit it. thank you gerry this is my first time, and if I excluded anything, I apologize. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law.
