Add QR code output to PHP script already producing PDF files

Write a simple PHP script that outputs a PDF file with a single line of text and a QR code (pointing to a URL) on it. I would like to have the script take the values to produce the text and the QR code from within the script itself (that is, I will manually input the values into variables in the script when I run the script, such as $textToOutput - "Hello World" and $urlTarget = "[login to view URL]"). My server is set up to use PHP QR Code libraries ([login to view URL]) for the generation, so the QR code generation must use this library. This should be very quick for someone who is familiar with the PHP QR code library.
