Convert screen to Sencha/Ext JS

This is a trial run...we have MANY screens in our application that need to be converted to Sencha/Ext JS. At this point, we're really looking for UX or design work to layout the screen in a more usable, more readable, touch friendly layout using Ext JS. There are multiple tabs on this customer profile screen, and many of the fields are WAY too long/big. I'm okay with grouping, consolidating, removing tabs, etc. Needs to be quick and friendly, and work on a tablet. I don't like how the address and phone are on different tabs. It is inefficient and is not easy to navigate.
Please submit a screen shot of how you would architect the attached image using Sencha Ext JS and make it more user friendly. If I select your proposal, we have MANY more screens to design from here.

I have worked on EXTJS. I know it can be done easily with the use of Tabs, few proxy methods, some dynamic controls.
