Convert WireFrame to one HTML webpage based on template

I need a web designer to design the HTML,CSS,JS , so I can share it with the back-end developer to implement it to the system.

And if I like the page I will assign the whole project to you .

Following are the requirements:

first we want to create the contact management page, it allow the user to manage his contacts data (like the contact list in your phone).

the filed required (for New Contact & view Contact)

- Contact name.

- photo.

- Company.

- Type(Business, Personal, Vendor)

- Category (School, Office, Militiry, family..etc)

- Phone.

- E-mail.
