Create 2 form data to email and setup paypal

This is an adult toy novelty site for a client of mine, and if it’s offensive, I do apologize and ask you to please disregard this project. I need someone that can send an order through paypal and when the order is confirmed then have form information emailed twice so that the data received in that email can be placed in two CSV files. I just need "ONE" page setup and i'll use what you create to setup all of the additional pages.
Once paypal returns, then the first customer email should be sent and received exactly in this format (you’ll see the data for the foms on the site):
FIRSTNAME,LASTNAME,ADDRESS1,ADDRESS2,CITY,STATE,ZIP,COUNTRY,CONTACTNAME,EMAIL,PHONE,,INVOICENUMBER,,UPS Ground
This info will be copied from the email and manually put directly into a CSV file.
The second product email should be sent and received in this format:
INVOICENUMBER,PRODUCTNUMBER,QUANTITY
The email format to be sent will make more sense when you look at [url removed, login to view] I created how the form would look that you would be creating to gather the info needed to be sent once paypal is done.
Please understand the majority of this description before bidding. Also, only "one" page need to be setup and i'll do the rest.
