Build Website with Forms and Online Payments with Paypal

Basically what I need is a web site with the same look at feel as our main web site. However, we are hosting an event that will need to host forms that can be filled out online and sent in via email. Each form when sent should have its own integrated subject for what it is relating to. Two of the forms involved need to be able to collect the data, calculate totals, and add to a shopping cart that can be paid for via Paypal while also sending a receipt to the sender as well as to the email address for the event. Will need to be designed similar to our web site as far as colors and looks.

Advertisements (Form with Paypal and Upload Option)

Please review one of the forms for our main web site link so that you know the colors. It can look just like our main site. I need this as soon as possible like within 3-5 days max once I have selected you as a vendor.
